Program Expectations Contract. Pretty straight forward. Additional details can be found in your Cucuyo Experience Handbook.
Notarized travel letter. Attached is a form letter that must be filled out and signed by both of your legal guardians (if applicable) and notarized. This is the only document that must be mailed and not scanned.
Travel Liability Waiver. Must be signed by parents.
Non-refundable program deposit. Your $500 deposit must be paid or postmarked by April 15th. You can make program payments by clicking the button to the right or send a check payable to Cucuyo Creative Explorations. The remaining $1700 balance is due May 15th.
Copy of your passport. If you do not already have a passport, you will need to begin that process immediately. You will need to purchase a $10 tourist visa upon arrival in Santo Domingo.
Copy of your health insurance card. All participants must have primary coverage. Cucuyo will provide you with iNext supplementary travel medical insurance. You will receive an email from iNext regarding instructions for receiving your iNext card.
Picture of you. So Cucuyo and your homestay can start getting to know you!
All documents except the travel letter (turn it in to your teacher) can be scanned and emailed to moreplease@cucuyo.org.
